A Message for All New Clients or Clients Who Have Not Been Seen for Awhile and Answers to Frequently Asked Questions:


We require that all new clients of the Harmony Center, the Spectrum of Rockford L.G.B.T.+ Counseling Center, Dr. Lisa Breisch, or Kristy Breisch, Licensed Massage Therapist conduct a brief intake over the phone during our business hours with our office manager.  Please allow for approximately 15 minutes of your time to conduct our intake process.


During the Telephone Intake, We Will Cover The Following:

  • Demographic Information For All Of The Client(s) Who Will Receiving Services
  • ​Reasons All Of The Client(s) Are Seeking Services
  • If You Have Ever Been A Client To Our Center 
  • Payment Arrangements That All Of The Client(s) Will Be Using
  • Verification of Insurance Benefits, E.A.P. (Employee Assistance Program) Benefits, or Our Reduced Self-Pay Fees
  • As A Courtesy to All Clients, We Will Verify Your Mental Health and E.A.P. Benefits Prior To Your Appointment
  • Scheduling Of Your First Appointment
  • Ability To Participate In Appointment Reminders Via Phone Message, Text Message, or Email Message
  • Reminder To Bring Identification Card(s) and Insurance Card(s) (if using insurance)
  • Notification To Arrive 1 Hour Prior To The Scheduled Appt. to Complete Required Forms (Forms Are Only Sent Out To Clients With Office Manager and Therapist Approval).  We Require Clients To Come In Person So That We Can Answer All Questions and Assist With Completion of Required Forms.  All Forms Must Be Completed and Signed Prior to the Start of Services.
  • Our Address, Contact Information, Directions To Our Center, and Nearby Landmarks
  • Mental Health Evaluation, Therapy, and Psychological Testing Sessions Last 45-50 Minutes
  • Reminder That Covid-19 Health Standards Are Enforced Even If Client(s) Have Been Vaccinated 
  • Parents/Guardians of Children/Teens Under Age 18 Must Be Present To Participate In Session and to Provide Clinical Updates
  • Cancellation Policy - We Require 12 Hour Notice of Any Cancellation or a Cancellation Fee of $25.00 Will Be Charged
  • Any Copay, Co-Insurance, Deductible, or Self-Pay Fee Is Expected Prior to the Start Of Session - We Accept Cash or Charge Only
  • We Reserve The Right To Cancel Your Appointment If Our Office Policies Are Not Followed


Things To Expect At Your First Appointment PRIOR To Meeting With A Mental Health Therapist:

  • Arrive 1 Hour Prior To Scheduled Appointment Time To Complete Required Paperwork.
  • Completion of Covid-19 Health Questionnaire and Temperature Taken by Forehead Scanner
  • If Late for Whatever Reason and the Completion of Paperwork Spills into the Scheduled Appt. Time with the Therapist, You Will Have a Shorter Session.  
  • Completion of All Required Paperwork Including Signatures of Parents/Guardians and All Those Participating In Services.
  • Completion of Our Biographical History Form Including Review of Mental Health History, Medical History, and Symptom Checklist.
  • Review of Our Office Policies With Our Office Manager Including Privacy Practices, Patient Rights, Cancellation Policy, Fee and Payment Policy, etc.
  • We Will Photocopy Participant(s) Identification Card(s), Driver's License(s), and Insurance Card(s) - Please Remember To Bring These With To the First Appt.
  • Completion of Release(s) of Information For Us To Obtain Records, Share Records, or Request Care For Mental Health Treatment
  • You Have The Right To Request A Photocopy of All Documents Completed


At Your Appointment WITH the Mental Health Therapist the Following is Covered During 45-50 Minute Mental Health Evaluation Session:

  • Any Questions About The Paperwork or Our Office Policies
  • Reasons For Seeking Help
  • How Long Problems Have Been Occurring
  • Symptoms Experienced
  • Mental Health History
  • Family Mental Health History
  • Review Of Medications - Over The Counter and Prescribed
  • Medical Problems That May Be Affecting Symptoms
  • Review of Substance Use - Caffeine, Tobacco, Nicotine, Cannabis, Alcohol, Illegal Drugs, Etc.
  • ​Any Other Providers the Client(s) Are Working With - Medical Doctors, Psychiatrist, Therapist, Probation Officers, Lawyers, etc.
  • Review of Your Coping Strategies
  • Review of Your Support System
  • Recommendation for Treatment by the Mental Health Therapist
  • Possible Homework or Things to Do or Read/Research That the Therapist Deems as Helpful
  • Answering of Any Questions 
  • Scheduling of Your Next Session with the Office Manager (Before or After Your Session)


Helpful Suggestions to Prepare for Future Mental Health Therapy Sessions with the Therapist:


  • Take Notes or Journal Thoughts, Feelings, Behaviors and Stressful Events That Have Occurred Between Therapy Sessions
  • Be Prepared to Talk About How You Have Been Coping and What Has or Has Not Been Helpful
  • Any New or Worsening Mental Health Symptoms
  • Any New or Worsening Medical Symptoms
  • Any Addition, Increase, Reduction, Discontinuation, or Change to Mental Health Medication
  • Any Addition, Increase, Reduction, Discontinuation, or Change to Physical Health Medication
  • Any Side Effects or Reactions to OTC Medications, Prescription Medications, or Herbal/Vitamin/Mineral Supplements
  • Any Thoughts, Plans, Means of Self-Harm, Suicide, Harm to Others, Homicide, Consumption of Alcohol or Drugs, or Dangerous/Risky Behaviors
  • Any Inpatient Medical Health or Psychiatric Mental Health Stays
  • Any Experiences of Mental, Verbal, Physical or Sexual Abuse/Assault
  • Any Change of Other Providers Such As Doctors, Psychiatrists, Therapists, School Social Workers, etc.
  • Any Contracture or Exposure to the Covid-19 Virus by Oneself Or the People Living WIth, Including Positive or Negative Tests, Quarantine, Vaccinations, and Symptoms
  • Any Crisis That the Client(s), Family/Friends/Significant Other(s) Living With, Family/Friends/Significant Other(s) Not Living Are Experiencing

Harmony Center for Holistic Psychotherapy is located off of N. 2nd Street (Rt. 251), on the service road,  in Loves Park, IL.  Our business is in a residential building, look for the yellow house with raspberry trim.  We are near the corner of N. 2nd Street and Windsor Road.  We are situated between Capable Cars and Ambrose Christmas Tree Store.
