NEW PATIENT INFORMATION AND REQUIRED DOCUMENTATION AND FORMS
When calling the Harmony Center for the first time as a new client, please allow approximately 15 minutes for our office manager, Kristy, to conduct a brief telephone intake to register you for services. We will ask you for the following information:
- Your name and the names of all people who will be participating in services (such as a spouse, children, partner, or other family members).
- Your date of birth and the date of birth of all people participating in services
- Your address and the addresses of all people participating in services
- Your insurance information, please have your insurance card handy
- E.A.P. Company if using E.A.P.
- Name of employer if using insurance or E.A.P.
- Your email address
- Reasons you are needing our services, these are the problem areas you would like help with
Kristy will then schedule your first appointment. She will also verify your insurance benefits such as your mental health deductible, copay, coinsurance, and the amount of money due at each session. If you are not using insurance, we offer a self-pay fee, which will be due at the time of your appointment. We ask that all new clients, and previous clients who have not been seen in 3 months or more, come in one hour prior to the scheduled appointment time to complete our opening paperwork packet. Kristy will assist you with any questions you have while completing the paperwork.

- Are you married and do you have children?: Married to my lovely bride, Kristy, and I have three awesome adult step-children.
- Are you a pet lover?: Absolutely! Our center is pet friendly (pets must be on leash or in container and well behaved). Emotional support animals are welcome. Need help with a letter to obtain certification or approval for an emotional support animal? I am happy to accommodate your need! Dr. Lisa and Kristy have several fur and feathered pet babies, and Dr. Lisa has been a various species pet lover since a young child. Dr. Lisa also specializes in pet grief and loss and has provided therapy groups in the past for those in need.
- How long will it take for me to get better?: Everyone's healing journey is very personal and individual-needs-specific. Healing timelines vary from individual to individual. Your time, energy, and dedication to your healing journey play very important factors in getting better. I encourage all clients to include their family and significant other in the therapy process to help increase their support and their understanding of how to help you. Dr. Lisa has an "open door policy" which means that you are welcome to bring anyone with you to your session as long as you feel it would be helpful to your healing journey. Dr. Lisa will also consult with any other treatment providers that you are working with as long as we have a release of information form giving your permission to do. This can include other therapists, psychiatrist, medical doctors, nurse practitioner, FMLA/Disability company, lawyer, school counselor, learning specialist, and community agencies. Having a "treatment team" that is all on the same page is very important to your care and treatment timeline. Additionally, using a variety of traditional and holistic-integrative strategies is an approach we take, and have found that being a fully person-centered - MIND-BODY-SPIRIT - clinic helps clients to heal on many levels. A person's mind-body-spirit is all connected, having one issue in one area will significantly affect the other areas. Addressing needs on all levels allows a person to fully heal.
- What happens if my insurance coverage stops? We do offer a self-pay fee for people who have lost their insurance coverage and who may be in between insurance plans.
- What is your session cancellation policy?: We ask that you notify us as soon as you know that you will not be able to make it to your session. We ask at the minimum that you notify us 12 hours prior to your appointment so that we may reschedule your next appointment as soon as possible, that we arrange with you any needed changes to homework or treatment, that we can offer your appointment time to another client as soon as possible as we do have people on our waiting list in case a sooner appointment becomes available. If you notify us at the last minute or simply forgot your appointment (no call and no show to the appt.), we do charge a $25.00 missed appointment fee.
- If I am having a psychiatric emergency and am feeling suicidal or my mental health symptoms worsen, how soon can I be seen for an appointment?: If you are a current client, please contact our office IMMEDIATELY as Dr. Lisa can provide an emergency phone session the same day if she is booked with appointments. If she is not booked, she can offer you a regular in-person session during business hours the same day (3pm-10pm). Dr. Lisa provides her cell number and asks that you also contact her via text message before or after business hours. She may be in a place where she cannot hold a confidential call so texting first is the safest option.

- What types of massage and bodywork services do you offer?: Many! Please click on the Massage Therapy tab for a list of massage therapy and bodywork services offered by Kristy.
- How long have you been a massage therapist?: Since 2011, for the past 11 years.
- Where did you learn massage therapy?: Rockford Career College, Rockford, IL.
- I'm new to massage therapy and bodywork, so what should I expect?: Kristy will provide you with a health intake and ask you about areas on your body where you are experiencing pain, tension, or discomfort in. She will recommend services that may be helpful to you. Kristy can tailor your massage and bodywork experience to your specific needs and preferences.
- I'm new to massage therapy and I'm afraid it will hurt: Kristy can provide light pressure, medium pressure, or deep pressure. Your feedback is essential to your treatment and Kristy will frequently check in with you about your desired level of pressure as it may be different on different body areas. For example, you may want a relaxation massage without deep tissue work. Or you may need deep pressure on your feet but light pressure on your lower power.
- I'm self-conscious about being naked: No one is expected or required to be naked. Clients can remove clothing down to underwear or can choose to keep all clothing on. Kristy will professionally drape you with a bed sheet and blanket and only expose the area of the body that you request to be worked on, one body section at a time, while the rest of your body is covered by sheets and a blanket.
- I'm self-conscious about having extra weight: No worries about this aspect! Kristy's massage bed is made for all bodies up to 600 lbs. Kristy is also specially trained in working with plus-size bodies and all staff at the Harmony Center are size-friendly and accepting of all body shapes and sizes. Besides, both Kristy and Dr. Lisa are plus-sized and personally understand the joys and difficulties associated with having a larger body.
- I'm self-conscious about being trans: No worries about this aspect! Kristy specializes in the massage needs of the L.G.B.T.Q.I.+ community. No matter if you identify as transgender, non-binary, genderqueer, etc. -- all body types, sizes, and shapes are accepted and valued at our center!
- I have medical issues and I'm not sure if it would be safe for me to have a massage or bodywork done: If you are interested in massage and bodywork services, simply call Kristy to discuss your concerns and to determine the services best suited for your medical conditions.
- I'm a trauma survivor and I don't know if I would be triggered by touch: Please make your needs known and Kristy will do everything she can to accommodate you. She has an extra chair in the room in case you would like a spouse or family member to be present during your bodywork. If you are also working with Dr. Lisa, mental preparation via trauma treatment with bodywork can be a very good option to learn healthy safe touch. You are always in control of your treatment at all times! Many clients of both Dr. Lisa and Kristy will schedule back-to-back sessions, seeing one of us first and then the other. Dr. Lisa can also make recommendations about chakra work and essential oils with Kristy that would be helpful to you based on your mental health needs. Dr. Lisa and Kristy are happy to work together for you as part of your treatment team!
